I certify that I am a citizen or lawful permanent  resident of the U.S., if applying for Voting Membership. I have read the Statement of Organization and Program Activities (the "Mission Statement") of the Association and, without qualification, if applying for Voting Membership, subscribe to its goals and any legal means the Association uses to achieve the same. If applying for Associate Membership, I (we) am (are) in sympathy with the purposes of the Mission Statement.  I am 18 years of age or older, or currently enrolled as either a cadet in an accredited college or university R.O.T.C. program, or as a cadet or midshipman at a U.S. Armed Forces Service Academy.

A person who is either an American citizen or lawful permanent resident and a war veteran of the Armed Forces of the United States of America as defined by Congress (see Qualifying Service above). A war veteran voting member applicant shall be have been enlisted, inducted or commissioned into any branch of the U.S. Armed Forces, including its Reserve and National Guard components, and the U.S. Merchant Marines, provided that such applicant who has served in the U.S. Merchant Marines served at least 90 days between December 7, 1941 to December 31, 1946, including service in the U.S. Army Transport Service or the Naval Transport Service; and who has served at least 90 days on active duty during "wartime" (as defined by Act of Congress); and who now has a discharge other than dishonorable or is eligible therefor. An individual who has served less than 90 days on active duty but was discharged because of a disability incurred in the line of duty is also eligible as an applicant for war veteran voting membership. A person who is on active duty and assigned to any branch of the U.S. Armed Forces, including its Reserve and National Guard components, shall be admitted to membership.

If applying for war veteran voting membership, I certify that I meet the requirements therefor as established by Act of Congress.

A certain number of voting membership open slots are reserved for civilians who are neither veterans nor war veterans, and who otherwise meet the qualifications of the certifications of this Application.

Membership Applications are processed on a first come, first served basis, except that Applications from active duty and former USN, USNR, USMC and USMCR personnel, including R.O.T.C. cadets and USNA midshipmen, are processed first.

I understand that there are no personal notices, benefits or rewards to membership, other than knowing any dues or contributions are applied to the stated purposes of the Association, or any other notices, benefits or rewards the Articles of the Association may provide or the National Executive Board may decide upon in any given year, and that the current rights and benefits of membership, which are subject to change without notice, are currently posted on, and will continue to be posted on, the Membership Page of the Association's website, in the Membership and Contributions Section.  I also understand that the Association expects every voting member to put in a substantial amount of quality time each year as a volunteer on Association outreach and/or administrative activities.

Full voting members may also inquire as to the details of current membership rights, responsibilities and benefits at any time by emailing the Association at USN@NavyVets.org or by telephoning the Association at (202) 736-1725.

I also understand any intentionally false statement on the Application is immediate grounds for termination of this annual, or any subsequent membership, without refund of any annual dues paid.  If signing for an Entity Associate Membership Application, I have the authority to do so.
